PMS or Anxiety? How to Tell the Difference

Is your anxiety caused by your period, or is it an underlying anxiety disorder? Learn how to distinguish between PMS-related mood changes and general anxiety.

Author: Dr. Nithanth Balshyam

Because anxiety is a common symptom of Premenstrual Syndrome (PMS), it can be challenging to determine whether your feelings of nervousness are driven by your cycle or if they point to an underlying anxiety disorder.

Distinguishing between the two is key to finding the right treatment and regaining control over your mental well-being.

Timing is Everything

The biggest indicator of whether you are experiencing PMS-related anxiety or a generalized anxiety disorder (GAD) is timing. PMS anxiety typically begins 7 to 10 days before your period and resolves almost immediately once bleeding starts.

If your anxiety persists throughout your entire cycle, regardless of whether you are menstruating or ovulating, it is more likely related to an underlying anxiety disorder rather than just PMS.

Premenstrual Exacerbation (PME)

It is also possible to have both. Premenstrual Exacerbation (PME) occurs when an existing anxiety disorder worsens significantly in the days leading up to a period due to hormonal shifts.

Keeping a detailed daily mood and symptom diary for two to three months can help you and your healthcare provider identify patterns and determine the best holistic treatment strategy.
